Parents Authorization

I hereby certify that the above list is true. I consulted with my child's doctor and he/she concurs that my child is physically fit and capable of participating in soccer activities. I give permission for the camp staff to seek medical attention if needed. I am responsible for any cost or treatment except for that provided by the camps excess medical coverage policy. I waive and release the entire staff of Jessica Gonzalez's Soccer-Strength and Conditioning Camp, Mercer County Park, Windsor Athletic Club, and Windsor Academy for damages, injuries, loss of personal property. I approve any photos of my child for future marketing and brochures for camps. By typing your name in the box below you are bound by this agreement and acknowledge that you have read and understand these terms and conditions.

**There will be absolutely no refunds. If you cannot make this years camp a 50% credit will be applied to future camps
